This order form is for your information.  It gives you all the information I need to correctly fill your order.  Please print out the Print Version and fill it out using the instructions on this page.  I have decided against using Pay Pal as their forms do not allow for the amount of information I need to fill your orders.  Please forgive the inconvenience. I am, however still working on it.
